The Duke Neurology Department provides verification of training for those who have completed our residency or fellowship programs. Beginning October 14, 2024, verifications for residency or fellowships completed more than one year after their graduation date will be charged a fee.
Fees for verification:
$50 for standard verification letter requesting the following:
- Name of Trainee
- Training Program/Specialty
- Dates of training
- Confirmation of completion of training or explanation of further information if program is not successfully completed
$75 for detailed verification of training, which includes:
- Standard verification information above plus
- Detailed verification of specific privileges and/or procedures requested
- Information from previous evaluations if requested
Instructions for requesting training verification:
- Complete the Qualtrics Verification Request Form. Once the request form is received, you will be sent an invoice for payment.
- Make payment.
- Email the following documents to NeuroVerification@duke.edu
- Authority to Release information signed by the trainee
- Your verification form
Once payment is confirmed and above documents are received, we will complete the verification request. Payment is charged for each verification completed.
Please allow 7 to 10 business days for this request to be processed after payment is received.
